This is my brain's cortical surface, reconstructed from a research MRI with FreeSurfer 7.4.1. It's an anatomical visualization, not a medical image or a diagnosis. The backdrop can show a live 3D model of the cortex, slices of the T1 MRI (brain only, resampled to 1.5 mm), or a still image. The Explore page adds deep structures and a movable MRI slice plane. Every cortical region is also listed as text on the About page.

Research / case study

One brain, twenty pipelines: how much do open-source neuroimaging tools agree?

independent2026

I ran one T1-weighted MRI of my own brain through about 20 open-source segmentation tools and models, then compared what they report.

Question
How consistent are widely used open-source segmentation tools on a single T1 scan, and can a general-purpose medical foundation model (SAM-Med3D) replace brain-specific tools?
My role
Independent project. I ran every pipeline and analysis on my own scan.
Data
One 1 mm isotropic 3D T1-weighted MRI (Siemens) from a 2026 research acquisition. n = 1.
Tools
  • FreeSurfer 7.4.1: recon-all, SynthSeg 2.0, SynthStrip, SAMSEG, ScLimbic, hypothalamic subunits, segment_subregions (hippocampus/amygdala, thalamus, brainstem)
  • ANTsPyNet: brain extraction, DeepAtropos, DKT, HippMapp3r, DeepFlash, cerebellum, claustrum, SHIVA PVS, lesion, DeepBrainNet
  • SynthBA
  • PCNtoolkit normative models (Rutherford et al., eLife 2022)
  • SAM-Med3D-turbo

In plain language

Brain-imaging software turns an MRI scan into measurements, such as how large the hippocampus is or how thick the cortex is. Many free tools do this, and research papers often report numbers from just one of them. I wanted to see how much those numbers depend on which tool you pick. So I took one scan of my own brain, ran it through about twenty tools, and compared what they said. The short answer: for small structures, the choice of tool matters a lot, and a general-purpose AI model for medical images was not a substitute for brain-specific tools.

Pipeline

  1. 01 · Input

    • One T1-weighted MRI
    • 1 mm isotropic, Siemens
    • n = 1
  2. 02 · Tools

    • FreeSurfer 7.4.1 (recon-all, SynthSeg 2.0, SynthStrip, SAMSEG, ScLimbic, subregions)
    • ANTsPyNet (10 models)
    • SynthBA
    • SAM-Med3D-turbo
    • PCNtoolkit normative models
  3. 03 · Comparisons

    • Structure volumes across tools
    • Dice against atlas labels
    • Sensitivity to calibration site
    • Quality control
Analysis pipeline: one scan in, about twenty tools, four kinds of comparison.

Findings

These are method-level results. The site shows no health information about my brain (see methods).

  1. Tools disagree most on small structures. The largest and smallest amygdala estimates differed by about 2.7×. For most structures, the direction of left–right asymmetry flipped between tools, so a number from a single tool needs uncertainty attached.
  2. SAM-Med3D wasn’t a substitute here. Against atlas labels after 5 prompts, it reached a Dice score of 0.82 on the brainstem but under 0.06 on the lateral ventricle, third ventricle, and caudate, where it segmented the surrounding tissue instead. On its own CT sample it reached a mean Dice of 0.68 from one prompt, which suggests the setup itself was working.
  3. A possible software issue. In FreeSurfer 7.4.1, SynthSeg’s --parc --vol output wrote cortical parcel volumes that sum exactly to total cerebral white matter instead of cortex. I used label-map voxel counts instead, which sum to within 1.3% of its cortex estimate. Status: observed on one scan. I haven’t called it a bug; the next step is to reproduce it on FreeSurfer’s sample subject bert.
  4. Normative scores depend on calibration. With no reference data from the same scanner, the choice of proxy site shifted z-scores by a median of 0.44 SD, and by up to about 2 SD for mean cortical thickness. Single-subject normative claims need same-scanner calibration.
  5. Quality control passed. Automated QC was clean, and the surface topology was clean (Euler number −32; the usual problem threshold is about −217).
